If your body shape has changed during perimenopause (particularly around your middle) you’re not imagining it. There are real hormonal and metabolic shifts happening behind the scenes. It’s not just about willpower or calories.
Oestrogen
Oestrogen helps determine where fat is stored. When levels begin to drop, the body tends to shift fat storage from the hips and thighs to the midsection.
Once periods stop, your fat tissue actually starts producing oestrogen (specifically a weaker form called estrone). The more fat tissue you have, the more oestrogen is produced, which sounds helpful, but can increase inflammation and affect how well your body manages blood sugar. Eating fibre-rich foods can help with this.
Insulin resistance
Falling oestrogen can also make your cells less responsive to insulin, the hormone that helps move glucose from your blood into your muscles and liver. When your body becomes more resistant, it pumps out extra insulin to compensate.
Higher insulin levels encourage more fat storage, particularly deep inside the abdomen around your organs (called visceral fat). This type of fat is metabolically active and can make insulin resistance worse, creating a vicious cycle. Eating lower-carb options can help with this.
Cortisol (stress hormone)
Chronic stress raises cortisol, which promotes abdominal fat gain. Visceral fat cells have more cortisol receptors, meaning stress hormones act more strongly there. Ongoing stress can also disrupt sleep and appetite regulation, both of which can drive further weight gain and inflammation. Be aware that the body can feel stress from fasting, so if you are a chronic faster, rethink this.
In Summary
Perimenopause shifts your internal chemistry. Lower oestrogen, rising insulin, and higher cortisol work together to promote fat storage in the belly region. You need to eat 3 meals a day (no fasting), keep things lower carb to help with insulin and make smart snack choices (protein and fibre options are great).
